Infradiaphragmatic - Suprahepatic Ectopic Kidney in 106-Year-Old Woman

Abstract

Objective: We report the case of a 106- year-old female with infradiaphragmatic and suprahepatic ectopic kidney. The diagnosis was incidental during the surgical management for a hip fracture.

Methods: We performed a bibliographic review on the incidence, origin, diagnostic, treatment and complications of this type of ectopic kidney.

Results:  X-rays of thorax with report of right basal opacity. An ultrasound scan was made. The results showed the right kidney positioned in the upper intercostal zone. Thorax CT Scan showed a normal infradiaphragmatic - suprahepatic right ectopic kidney.

Conclusions: Infradiaphragmatic - Suprahepatic ectopic kidney is a very rare finding. There are no reported cases of centenary patients in the literature reviewed.

Introduction

Ectopic kidney is a defect where the kidney is not located in the renal fossa. This is a rare congenital defect. It is asymptomatic in most patients. It is very common in children. Diagnosis in adults is usually incidental. There are few studies about the functional consequences of this congenital anomaly, especially in adults. In this studies data suggests no adverse effects on blood pressure or kidney function during childhood. There are few reports on adult patients, but none in long-lived, functional patients without any pathological precedent and without alteration in their renal function.

Case

We report the case of a 106-year-old female with the incidental diagnosis of infradiaphragmatic and suprahepatic ectopic kidney during the surgical management for a hip fracture. The patient is a 106-year-old female, who lived in the field through all her life. The patient was completely independent and cognitively normal without any personal pathological precedent. She never visited a doctor before this one time that she came to a hospital for a broken hip after a fall.

In her evaluation there were the following findings: X-rays of thorax with report of right basal opacity. An ultrasound scan was made. The results showed the right kidney positioned in the upper intercostal zone. Thorax CT Scan showed a normal infradiaphragmatic - suprahepatic right ectopic kidney.

Discussion

The anomalies related to an abnormal kidney elevation is called ectopic kidney. This anomaly includes alterations such as: pelvic kidney, which is most common, lumbar kidney and thoracic kidney, the latter being the most rare [1,2].

The ectopic kidney is formed by alterations in the womb during the first eight weeks of pregnancy, which is the time when the positioning of the kidneys takes place. The ectopic kidney diagnosis is incidental and the surgical treatment is given only if there are symptoms. Studies mention Ectopic Intrathoracic kidney as the rarest case. Nevertheless, there were no findings in literature about Ectopic Infradiaphragmatic kidney, especially in patients over one hundred years old [3].

Infradiaphragmatic - suprahepatic renal ectopic is a very unfrequent finding, there are no reported cases of centenary patients in the literature reviewed. Often asymptomatic, frequently an incidental diagnosis that does not require treatment.
